if you didn't know already. each pixel should be about 1 terrain sqare. and the brighter the pixel is the higher it will be. so like dont put a pixel thats like 188 188 188 in color beside one thats like 33 33 33 or you will get preaty bad surfaces.
after you done your bitmap you should use the blur and smudge tool to make the bitmap pixels look preaty smooth something like the attachment.
