The Buildings and Bridges that will not appear are not Server Side (came with the Game) and therefore you have to also include the .dif files in the .vl2. These Bridges came with some mappack or custom map download. Just copy and paste the .dif files in the interiors folder and make a new .vl2 file.
The damage Triggers for your fire has to have their DataBlocks loaded before the Trigger is loaded. You can look at the .mis file from my map "Omann Ru" to see how to load custom DataBlocks from the .mis file. It would be near the TOP of the .mis file.
As for the Lava Damage. Are you sure you set the waterType to Lava in the .mis file. That should work if you have it in the .mis file correctly. I like to just copy and paste Lava or Water from one .mis file to my .mis file using "Notepad" or similar program. I the make adjustments to the Lava later in the Editor.
